Job summary

Bridgewell in Massachusetts seeks a Licensed Practical Nurse to provide health services and direct care in a residential program for individuals with disabilities. The LPN will advocate and liaise with community health providers, monitor medical records, and administer medications following MAP policies.

The schedule is Wednesday to Saturday 11:00 pm–7:00 am, totaling 32 hours weekly. Candidates must have MA LPN license, a driver’s license, and be able to drive a 12-passenger van.

Qualifications

• Graduation from an accredited and approved licensed practical nurse program.
• Must have current Massachusetts Nursing license.
• Two years of general nursing experience and/or service to individuals with developmental disabilities in a community health setting is preferred.

Responsibilities

• Provide direct nursing and day-to-day care to individuals.
• Document health care services per state regulations and MAP policies.
• Collaborate with Residential Manager and RN for medical updates and care planning.
• Administer medications following MAP policies and procedures.
• Maintain medical charts and perform quality assurance reviews of medication systems.
