Job summary

Bridgewell is seeking a licensed practical nurse to support Day Habilitation Services in Massachusetts. You will coordinate medical services with prescribers and residences, provide direct healthcare, and document medically required information.

The role involves leading group activities and ensuring compliance with regulations. The position requires a MA LPN license, a background with individuals with disabilities, and solid organizational and communication skills.

Qualifications

• A minimum of a High School Diploma or GED is required.
• Current Massachusetts LPN license is required.
• One year of experience working with individuals with disabilities is preferred.
• One year of experience in a community setting is preferred.
• Knowledge of Day Habilitation Regulations and CARF standards is a plus.
• Strong organization, communication, and problem-solving skills are essential.

Responsibilities

• Coordinate medical services with each recipient's primary care physician, clinic, or residence.
• Provide healthcare services to all individuals in care.
• Complete medical documentation such as seizure reports and treatment charts.
• Lead groups (e.g., Hygiene, women's or men's groups) as needed.
• Provide in-services and training to staff on medical topics monthly.
