Indiana University Health is seeking a pharmacist to provide comprehensive, value-based medication care in collaboration with the care team. The role focuses on safety, effectiveness, and optimizing therapy through medication monitoring and regulatory compliance.

The position requires a PharmD or PharmD-equivalent, Indiana licensure, and may involve PGY1/PGY2 training, weekend rotations, and potential travel to IU Health facilities.

Qualifications

• PharmD or BS in Pharmacy (before 2006).
• ASHP PGY1/PGY2 residency may be required.
• 0–3 years of relevant experience.
• Current Indiana Pharmacist license or eligibility.
• Board certification or other specialized training may be required.
• BLS certification through AHA may be required.
• Travel to IU Health facilities may be required.
• Weekend rotations may be required.

Responsabilités

• Provide comprehensive, value-based medication-related care to improve patient outcomes.
• Collaborate with physicians and nurses to optimize therapy and ensure safety of medication use.
• Process and dispense medications per orders or prescriptions.
• Perform medication therapy monitoring and ensure regulatory compliance.
• Maintain security and confidentiality of medications and patient information.
• Provide medication information support to medical and nursing staff and ensure continuity of care.
