Maxim Healthcare is seeking an LPN to deliver 1:1 home-based nursing care for patients in the Uniondale area. You will provide direct patient care in patients’ homes, ensuring timely documentation and communication with the health care team.

Responsibilities include monitoring patient conditions, administering medications, and supporting individualized care plans while benefiting from flexible scheduling and continuous clinical support in a community setting.

• Active LPN license is required.
• BLS certification is required.
