Responsibilities

• Health & Wellness: monitor health, assist with medications as prescribed.
• Daily Living Support: help with ADLs and daily tasks.
• Goal Achievement: support participants to reach personal goals and build independence.
• Community Engagement: accompany outings and encourage social interaction.
• Documentation: maintain progress notes and paperwork accurately.
