JOB TITLE : Outreach Therapist

EMPLOYMENT TYPE : Full-time, 40 hours / wk

SCHEDULED HOURS : Monday - Friday, 8 : 30am - 5 : 00pm, rotating on-call and rotating holidays (additional compensation provided).

ABOUT THE PROGRAM : ASSERTIVE COMMUNITY TREATMENT TEAM (ACTT)

Assertive Community Treatment (ACT) is an evidence-based practice that improves outcomes for people with serious mental illness who are most at-risk of psychiatric crisis and hospitalization and involvement in the criminal justice system. ACT uses a multidisciplinary team approach combined with assertive outreach in order to provide a wide range of services to clients; including teaching of life skills, vocational counseling, substance abuse counseling, medication management, therapy, assistance with accessing health care and social and recreational activities.

POSITION HIGHLIGHTS

  • Facilitate a weekly clinical group and participate in community-based services including home visits, court, agency, and medical appointments.
  • Collaborate with a tight-knit, multidisciplinary team-treatment plans are developed together, with daily morning meetings and weekly supervision.
  • Enjoy a flexible, self-managed schedule with autonomy in your work, while also participating in an on-call rotation with additional compensation.
  • Work in a program where clients age with the team, allowing for deep, long-term therapeutic relationships.

Duties & Responsibilities :

  • Conducts Clinical and Risk Assessments, including : LOCUS, Co-occurring Screens, DLA-20.
  • Collaborates with individuals to create individualized, person-centered treatment plans and documents clients progress during treatment.
  • Provides individual and group therapy.
  • Use Motivational Interventions to assist clients in meeting their goals and moving along their paths to recovery, utilizing stage wise interventions.
  • Provide psycho-educational rehabilitation service for recovery from psychiatric and substance use disorders.
  • Support individuals to link and engage with natural (family, friends) or community supports.
  • Develop safety plans for clients and work with other staff to respond to crisis situations as needed.
  • Provide skills teaching, such as assisting clients in obtaining / maintaining housing and utilities; learning how to use transportation services, planning, and preparing nutritional meals, engage and communicate with family and friends, be safe in the community, care for children and other family members, and solve everyday problems that arise.
  • Provide intensive skill building interventions and support for self-management, self-advocacy, health and well-being, relapse prevention, and using community resources.
  • Participate in family education and support activities.
  • Maintain documentation in compliance with agency and program standards.
  • Participates in staff meetings, supervision, trainings, and other meetings as assigned.
  • Additional duties as required

QUALIFICATIONS :

Education : Master's degree in behavioral health field or related field.

Experience : 1+ years' experience working with adults with SMI and / or co-occurring illnesses; outreach experience preferred.

Licensure / Certification / Registration : LMSW, LPCA, LMFTA required if eligible; LCSW or LPC preferred.

COMPENSATION

53,372 / annually minimum, and up commensurate upon experience.

  • Actual rates are determined at the time of offer and are based off of relevant experience, as well as adherence to our internal equity policy. Many of our positions offer program / location differentials and / or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
