Program/Location: Assertive Community Treatment Team (ACTT), Manchester, CT
About the Program: Assertive Community Treatment Team (ACTT) serves adults 18 years of age and older, providing intensive community-based treatment, support, and rehabilitation for individuals with serious persistent mental illness (SPMI) or dual diagnosis of SPMI and Substance Use Disorder.

Duties & Responsibilities:
- Conducts Clinical and Risk Assessments, including: LOCUS, Co-occurring Screens, DLA-20.
- Collaborates with individuals to create individualized, person-centered treatment plans and documents clients' progress during treatment.
- Provides individual and group therapy.
- Uses Motivational Interventions to assist clients in meeting their goals and moving along their paths to recovery.
- Provides psycho-educational rehabilitation services for recovery from psychiatric and substance abuse disorders.
- Supports individuals with linking and engaging with natural (family, friends) or community supports.
- Develops crisis plans for clients and works with other staff to respond to crisis situations as needed.
- Provides skills teaching, such as assisting clients in obtaining/maintaining housing and utilities, learning how to use transportation services, planning and preparing nutritional meals, engaging and communicating with family and friends, and solving everyday problems.
- Provides intensive skill-building interventions and support for self-management, self-advocacy, health and well-being, and relapse prevention.
- Participates in family education and support activities.
- Maintains documentation in compliance with agency and program standards.
- Participates in staff meetings, supervision, trainings, and other meetings as assigned.
- Additional duties as required.
Qualifications:
Education: Master’s of Social Work.
Experience: 2+ years working with identified populations. Preferred: Experience working with families, providing family therapy; individual and group.

Compensation: $54,996/minimum, and up commensurate upon experience. Actual rates are determined at the time of offer based on relevant experience and adherence to our internal equity policy.
