Job summary

Key Point Health Services in Dundalk, MD is seeking a full-time Rehab Counselor to join the PRP Outreach team.

You will conduct psychoeducational groups, support ADLs, and coordinate treatment planning, documentation, and advocacy for clients.

Candidates should have a high school diploma with experience or an AA degree in a related field, and a Maryland driver’s license; driving 15-passenger vans is required.

Qualifications

• Knowledge of DSM and symptoms of common PRP diagnoses.
• Experience with assessments and creating treatment plans.
• Strong writing, interpersonal and oral communication skills.
• Sensitivity to diverse populations and confidentiality.

Responsibilities

• Conduct and document onsite and offsite psychoeducational groups.
• Teach community skills including ADLs as needed.
• Ensure timely and accurate documentation such as treatment plans and progress notes.
• Safely transport PRP members in 15-passenger vans.
• Provide comprehensive services: assessment, case management, psychoeducation, crisis intervention, advocacy, and community liaison.

Who We Need

We are currently looking for a full-time Rehab Counselor to join our Dundalk PRP Outreach team.

What We Need

• Someone who has knowledge of the DSM and relevant symptoms of the most common diagnosis within the PRP programs.
• Someone who can demonstrate proficiency in assessments, treatment planning, advocacy skills and case management skills.
• Someone who can demonstrate the ability to express themselves well through writing, interpersonal and oral communication skills.
• Someone who has an understanding and sensitivity to serving culturally diverse and/or special needs population while maintaining patient confidentially.

Key Responsibilities Include

• Conduct and document onsite and offsite psychoeducational groups.
• Responsible for teaching community skills including ADLs as needed.
• Ensure timely and accurate completion of all documentation such as treatment plans and progress notes. Staff are responsible for teaching community skills including ADLs as needed.
• Safely transport PRP members to and from the program and offsite activities.
• Responsible for the comprehensive services offered through on-site and off-site programs. This includes assessment, case management, psycho education, crisis intervention, advocacy, and community liaison.
• Must drive 15 passenger vans.

What You Need

• High school diploma AND 2 years of experience in psychiatric rehab, mental health or related human services field
• OR
• AA degree in psychology, sociology, psychiatric rehab, mental health, or a related human services field with no experience required
• Valid and active Maryland driver's license (or state of residence) (must be active for at least 3 years)
• Pass a criminal background check, MVA check, and provide a negative drug screen.
• Must have own vehicle.
