Job summary

AltaPointe Health is hosting Open Interviews for Outreach Specialists in Mobile, AL. The event will run 10:00 AM – 2:00 PM at EastPointe Hospital in Daphne, AL 36526. Interview with a Recruiter to discuss open positions and the care teams involved.

Responsibilities include crisis response, community-based visits, and coordination of care with AltaPointe programs. Citizens are guided on safety plans and may receive transportation and supervision as needed.

Qualifications

• Bachelor’s degree in a mental health discipline is required.
• Must be 23 years of age or older; valid Alabama driver's license with good driving record.

• Makes face-to-face visits in the community and to consumer homes as needed.
• Educates families on crisis de-escalation and safety plans.
• Assists in securing emergency services or law enforcement when needed.
• Communicates effectively with CarePointe and Alta Pointe treatment teams.
• Documents assessments and crisis resolutions per Alta Pointe policy.
• Manages caseload and participates in Performance Improvement activities.

Overview

AltaPointe Health is hosting Open Interviews on Thursday, August 27th from 10:00AM-2:00PM. This event will take place at EastPointe Hospital (7400 Roper Ln; Daphne, AL 36526). Please make sure your application is updated prior to this event. When you arrive you will be able to interview with a Recruiter who can talk about all our open positions.

Adult Outpatient Programs provide the least restrictive level of care where adults may receive clinical interventions aimed at decreasing existing signs and symptoms. Outpatient treatment focuses on recovery, problem resolution, symptom reduction, stabilization and/or prevention of movement to a higher level of care. Treatment varies widely and depends upon the consumer’s presenting symptoms.

Responsibilities

Outreach Specialists are responsible for responding to crisis calls, scheduling and attending face to face visits following calls to CarePointe or the Outreach Liaison, educating families on crisis de-escalation and safety plans, and coordinating care with appropriate AltaPointe programs.

Essential Functions

• Responds to crisis calls in a timely manner
• Makes face to face visits when calls come into CarePointe or the Outreach Liaison
• Provides services in the community, consumer’s homes, etc.
• Demonstrates the ability to recognize the elements of a crisis state and knows how to deescalate or resolve the situation
• Effectively determines the level of response needed in crisis situations
• Assists in contacting family members if needed
• Communicates effectively with CarePointe to assist in finding appropriate placement when necessary
• Evaluates the consumer for community referrals needed
• Educates families on de-escalating crisis situations including but not limited to safety plans
• Assists in securing emergency medical services or law enforcement if needed
• Advocates on behalf of the consumer to secure community resources
• Effectively collaborates with the treatment team members and outside sources in the delivery of services
• Follows up with consumers that are in AltaPointe’s Adult Outpatient Programs who may be decompensating or at high risk
• Travels to consumer’s homes to ensure medication compliance as needed
• Utilizes appropriate communication methods to ensure appropriate communication with individual consumers’ treatment teams
• Handles and triages calls from multiple referral sources
• Effectively demonstrates the application of adult specific competencies
• Provides transportation to services on a limited basis
• Seeks clinical supervision and consultation as needed
• Accepts and employs suggestions for improvement
• Actively works to enhance case management skills
• Documents in a timely fashion per Alta Pointe policy
• Documents using Avatar appropriately
• Documents appropriate, complete, and comprehensive assessments
• Documents in detail the resolution of crisis situations and linkage to appropriate follow up sources
• Documents in a clear and concise manner services provided before the completion of the shift
• Documents legibly
• Effectively manages caseload based on consumer needs

Qualifications

Bachelor’s degree in a mental health discipline. Must be 23 years of age or older; has and maintains a valid Alabama driver’s license with a good driving record and must have proof of current liability insurance.
