• Demonstrates the ability to recognize crisis states and deescalate or resolve situations.
• Evaluates consumers for community referrals needed.
• Travels to consumer homes to ensure medication compliance as needed.
• Documents in a timely fashion per Alta Pointe policy.
• Meets Alta Pointe productivity standards.
• Treats consumers with care, dignity and compassion.
