CVS Health is seeking an Outreach Coordinator to support Aetna Better Health of Louisiana (ABHLA) Quality Management and Population Health teams. The role focuses on improving healthcare outcomes, closing care gaps, and advancing performance on HEDIS and related measures.

The coordinator will liaise with members, providers, community partners, and internal teams to promote evidence-based care, preventive services, chronic disease management, and quality improvement activities.

Position Summary

The Outreach Coordinator supports the Aetna Better Health of Louisiana (ABHLA) Quality Management and Population Health teams in improving healthcare outcomes, closing gaps in care, and advancing performance on HEDIS, State Withhold Measures, Performance Improvement Projects (PIPs), and Population Health initiatives.

This position serves as a liaison between members, providers, community partners, and internal departments to promote evidence-based care, preventive services, chronic disease management, and quality improvement activities. The Outreach Coordinator utilizes expertise to educate providers and members, analyze quality performance data, facilitate outreach campaigns, support provider engagement efforts, and contribute to interventions that improve member health outcomes and satisfaction.

The Outreach Coordinator supports provider and member outreach initiatives, reviews clinical opportunities for improvement, collects and analyzes quality data, participates in quality interventions, and promotes evidence-based practices that improve healthcare quality, safety, access, and member experience.

• Medicaid, managed care, HEDIS, or accreditation experience preferred.

Pay Range

The typical pay range for this role is:

$21.10 - $49.08

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
