Job summary

GHHC is seeking a proactive outreach liaison to strengthen patient engagement and physician relationships across Westminster, CO. The role focuses on increasing patient retention, referrals, and community presence through proactive visits, targeted outreach, and data-driven strategies.

The ideal candidate has a healthcare or sales background, 3+ years in home health or related fields, and strong communication skills. Travel to patient sites and clinics is common, with a background check required.

Qualifications

• Bachelor's degree in Healthcare, Sales, or related field preferred.
• Minimum 3 years of experience in home health care, outreach, community engagement, or physician liaison roles.
• Proven success in relationship-building, patient retention, and referral generation.
• Strong communication, organizational, and problem-solving skills.
• Proficiency in Salesforce or similar CRM systems required.
• Offers are contingent upon passing a background check.

Responsibilities

• Conduct face-to-face home visits and phone calls to address concerns and reinforce GHHC's value.
• Ensure 100% of patients receive monthly/quarterly touchpoints.
• Identify at-risk patients via surveys, caregiver feedback, and proactive outreach.
• Utilize CQ/HME letters during visits to negotiate pay rates and prevent transfers.
• Implement retention plans with structured follow-ups.
• Develop outreach strategies to boost brand awareness and patient acquisition.
• Build relationships with local unions, community organizations, and stakeholders to generate referrals.
• Contact patients after intake to schedule physician appointments with supportive PCPs or specialists.
• Present causation packets and educate providers on DOL requirements and response types.
• Onboard new supportive MDs by attending initial appointments and establishing communication preferences.
• Obtain new patient referrals from physician/clinic relationships.
• Integrate patient engagement, community outreach, and physician relationship management to enhance retention and increase intakes.
