Aspire Health and Community Services in Massachusetts is seeking an Outreach Case Worker to support adults in the community who face chronic behavioral health challenges. This full-time role involves direct outreach, coordination with a multidisciplinary team, and ensuring access to services and resources.

The Outreach Caseworker position is a full time, outreach position providing services to Adults residing in the community, experiencing chronic behavioral health issues. (Diagnoses may include, substance use disorder, schizophrenia. dual diagnosis, schizoaffective disorder, physical issues, etc.)
The Caseworker works with a multi- disciplinary team that includes Master level clinicians, nursing staff, peer specialists and others involved in client care.
Compensation is based on candidate experience, qualifications, and certifications and licenses. Annual ranges listed are based on a full-time, 40 hours per week position.
Salary Range
$22.40 - $25.25 USD
