Job summary

The ELECT Outreach Worker at Allentown School District serves as a liaison among pregnant and parenting students, parents/guardians, local agencies and ASD supports. Responsibilities include outreach, ISP development, home visits, group meetings, and case management with referrals and follow-up to support high school graduation and economic self-sufficiency.

Essential duties emphasize cultural responsiveness, knowledge of community resources, and data collection.

Qualifications

• Bachelor's degree in education, social services, or related field.
• Experience in program management.
• Ability to work independently, and to interact well with others.
• Proficient in Microsoft Office programs, including PowerPoint, Word, and Excel.

Responsibilities

• Outreach to students, parents/guardians, agencies and community partners.
• Develop and implement Individualized Service Plans (ISPs).
• Conduct home visits, group meetings and case management with referrals and follow-up.
• Maintain accurate records and timely data collection and submission.
