The Night Ministry is a Chicago-based nonprofit that provides human connection, housing, and health care to individuals struggling with housing stability or poverty. Our work is centered around two core service areas:Outreach & Healthcare andYouth Housing. Through our Outreach & Health programs, we meet people where they are, on CTA platforms, in encampments, at libraries, and across city neighborhoods; offering consistent, compassionate care and vital health services.
Our Youth Housing programs provide shelter and supportive services to young adults, helping them build stability and work toward their goals in safe, affirming environments. This is a full-time position that requires availability on some evening shifts, including weekends as needed. Candidates should be comfortable working outdoors and on foot for extended periods during outreach.
You'll provide on-the-ground case management and follow-up care through the Health Outreach Bus, the CTA outreach program, and other key outreach locations. By fostering trust and reducing barriers to care, you'll help clients take meaningful steps toward housing and health stability.
Delivering case management and referrals through direct outreach at CTA locations, the Health Outreach Bus, and the Harold Washington Library.
Building and maintaining strong relationships with community referral partners (e.g., housing, medical, mental health, addiction recovery).
Completing timely and accurate documentation in systems such as Electronic Health Records (EHR) and Efforts to Outcomes (ETO).
Maintaining up-to-date case files in compliance with funder and agency requirements.
Participating in street-based outreach, including evening and weekend shifts, in various weather conditions.
Representing The Night Ministry at meetings, trainings, and public-facing events.
Supporting volunteer engagement during outreach and fostering a welcoming team dynamic.
Driving agency outreach vans as needed.
You're someone who blends professional boundaries with deep compassion. You're adaptable, detail-oriented, and committed to supporting people with respect and consistency, even in challenging conditions.
Bachelor's degree in social services or related field, or equivalent experience.
2–5 years of experience in outreach, case management, or health-related services.
Valid driver's license, clean driving record, and access to a vehicle.
Strong verbal and written communication skills.
Familiarity with trauma-informed care, harm reduction, and LGBTQ+ competency.
Comfortable working outdoors for extended periods in all seasons.
Are bilingual in Spanish and English.
