What to Expect (Job Responsibilities):
- Coordinate follow-up care plan needs for members by scheduling appointments or enrolling them in programs
- Assess compliance with medical treatment plans via telephone or on-site visits and identify barriers to compliance
- Recommend treatment plan modifications and determine the need for additional services in collaboration with case management and providers
- Identify opportunities impacting quality goals and recommend process improvements
- Establish and maintain relationships with local, state, or federally funded programs

What is Required (Qualifications):
- High School Diploma or equivalent
- Minimum of 1 year of related experience or an equivalent combination of education and experience
- Ability to assess compliance with treatment plans and identify barriers to health
- Strong communication skills for coaching members on health risk reduction
- Ability to prepare reports documenting case and compliance updates

How to Stand Out (Preferred Qualifications):
- Certified nurse assistant or certified medical assistant and/or BS/BA degree in a related field
- Bilingual or multi-language skills
- Experience as a Community Health Navigator
- Clinical experience in a healthcare setting
- Familiarity with local, state, or federally funded health programs
