Title: Outreach Care Specialist - CareBridge -100% Virtual

Work Environment: This role is fully virtual, requiring only in-person training sessions, offering flexibility and promoting work-life balance.

Role Overview: The Outreach Care Specialist ensures members follow treatment plans for less complex cases and seeks ways to improve their health and meet quality standards.

Key Responsibilities
  • Coordinate follow-up care plans, schedule appointments, and enroll members in programs.
  • Assess compliance with medical treatment plans via calls or visits, identify barriers, and coordinate resolutions.
  • Identify quality improvement opportunities and suggest process enhancements.
  • Recommend treatment plan modifications and coordinate additional services with case management and providers.
  • Refer members to local, state, or federally funded programs.
  • Coach members on reducing health risks.
  • Document case updates and compliance reports.
  • Build and maintain relationships with relevant agencies.
Minimum Requirements
  • High school diploma or equivalent, with at least 1 year of related experience, or an equivalent combination of education and experience.
Preferred Skills and Experience
  • Certified Nurse Assistant or Medical Assistant certification, or a related bachelor’s degree preferred.
  • Bilingual skills may be required for certain units.
  • Experience as a Community Health Navigator and clinical background preferred.
Additional Information

Position level: Non-Management, Non-Exempt.

Work shift: Full-time, with flexible remote work arrangements.
