Job description

Outreach Care Specialist, 100% Virtual, CareBridge

CareBridge Health, part of the Elevance Health family, aims to enhance health, independence, and quality of life for individuals in home and community settings through home-care and community-based services. The Outreach Care Specialist ensures adherence to treatment plans for less complex cases and proactively seeks ways to improve member health and meet quality goals.

Responsibilities include coordinating follow-up care, scheduling appointments, enrolling members in programs, assessing compliance via phone or on-site visits, identifying and resolving barriers, suggesting process improvements, modifying treatment plans in collaboration with case management and providers, referring to funded programs, coaching members on health risks, documenting updates, and maintaining relationships with agencies.

The candidate must reside in the Eastern or Central time zone. This is a 100% virtual role, with in-person training sessions required. Shift options are 8:00am-5:00pm EST or 8:00pm-5:00pm CST. Minimum requirements include a high school diploma and at least 1 year of related experience or equivalent education and experience. Preferred qualifications include CNA or CMA certification, a BS/BA degree, strong communication, problem-solving, facilitation, analytical skills, and bilingual abilities if applicable. Salary ranges from $18.31/hr to $34.69/hr. Benefits include comprehensive packages, incentive programs, stock purchase options, and 401k contributions, subject to eligibility.
