Job summary

Neighborhood Healthcare is seeking an Outreach and Enrollment Counselor to educate and assist clients enrolling in affordable health insurance through Covered CA, Medi-Cal, Medicare, and CHIP, plus programs like CalFresh. The role supports community events and partnerships to expand access to care.

Qualifications

• High school/GED required.
• Covered California certification required within 90 days of employment.
• One year of experience in government or community program eligibility/screening for Medi-Cal or Medi-Cal for families preferred.
• Certified Enrollment Counselor for Covered CA experience preferred.
• Valid CA driver’s license and proof of auto insurance required.
• Bilingual (English/Spanish) highly preferred.

Responsibilities

• Conduct outreach and presentations at schools and community events (health fairs, churches, farmers markets, local businesses).
• Network and establish community relationships and partnerships.
• Monitor monthly/yearly goals and complete data for reporting under grant contracts.
• Support patient retention and growth of new patients.
• Present health concepts to individuals and small groups.
• Disseminate information about affordable health insurance coverage to families and individuals.
• Make in-reach phone calls to current patients to offer enrollment assistance.
• Provide enrollment assistance for Covered CA, Medi-Cal and CHIP as appropriate.
• Provide education and enrollment assistance for CalFresh.
• Educate on other resources such as SDG&E CARE program.
• Assist applicants with questions about health insurance and enrollment.
• Refer applicants to medical home and resources at time of enrollment.
• Receive referrals from partner agencies.
• Coordinate inter-department referrals for needed services.
• Provide coverage updates about health insurance.
• Attend meetings/training for role sustainment and growth.
• Prepare monthly reports on outreach/in-reach and application assistance.
• Share patient feedback to improve access to healthcare.
• Promote organizational activities and mission.
• Refer patients to Medicare agents as required.
• Assist in retaining patients and reducing attrition.

The Outreach and Enrollment Counselor will conduct outreach and in-reach, educate, and assist clients enrolling in affordable health insurance coverage through Covered CA, Medi-Cal, Medicare, or the Children’s Health Insurance Program as well as non-health insurance programs such as Cal Fresh.

Work schedule: M-F 8am-4:30pm. This position requires availability to work evenings and weekends as needed to support community events/meetings. While the standard schedule for this role is 40 hours per week, flexibility is provided and weekday hours can be adjusted when weekend/evening work is required.

Responsibilities

• Conducts outreach activities and presentations at school and community events such as health fairs, churches, farmers markets, grocery stores, and local businesses; events may be scheduled during weekday, evening or on weekends
• Networks and establishes community relationships and partnerships
• Understands and complies with monthly and yearly end goals and objectives in the various grant contracts and maintains complete data for monthly reporting
• Supports NHC efforts for patient retention as well as increasing new patient growth
• Presents health concepts to individuals and small groups
• Disseminates information about affordable health insurance coverage to families and individuals
• Conduct in-reach phone calls to current patients to offer enrollment assistance
• Provides enrollment assistance in affordable health insurance coverage through Covered CA, Medi-Cal and the Children’s Health Insurance Program as appropriate
• Provides education and enrollment assistance for the CalFresh program
• Provides education on other community resources and assistance programs, such as SDG&E CARE program
• Assists the applicant with questions regarding health insurance, healthcare access and other resources and ensures enrollment process is completed
• Refers applicants to their medical home and other needed resources at time of application completion
• Receives referrals from collaborating and partner agencies
• Performs inter-departmental referrals for patient in need of medical, dental, and behavioral health services or other available services
• Provides coverage updates to clients regarding their affordable health insurance
• Attends meetings and trainings necessary for the position sustainment and growth
• Prepare monthly quantitative and qualitative report on outreach/in-reach and application assistance activities
• Shares feedback from patient experiences to improve access to health care
• Keeps self and others informed of organizational activities and promotes mission and goals
• Conducts patient referrals to assigned Medicare agents as required
• Assists in the efforts to retain patients and support efforts at eliminating patient attrition

Qualifications

Education/Experience

• High school/GED required

Pay range: $27.21-$38.10 per hour, depending on experience.

Compensation Disclosure: The posted salary range reflects the designated pay grade for this position. While this range represents the broader classification of the role, actual compensation will be based on several factors, including but not limited to: the candidate’s overall knowledge, skills, and experience, market data and industry benchmarks, internal equity within the organization, Budgetary considerations and organizational needs. As a result, placement within the range is not guaranteed, and the full pay grade range may not be utilized.
