Job summary

Community Care Physicians is seeking an experienced Nurse Practitioner or Physician Assistant to join Vascular Health Partners in New York's Capital Region. Work with four board-certified vascular surgeons in an outpatient-focused practice with a cutting-edge interventional suite and on-site noninvasive vascular testing.

The role includes travel to satellite offices (Saratoga Springs, Queensbury, Amsterdam) to provide outpatient vascular care, with a four-day workweek (4-10) and dedicated admin

Qualifications

• Current New York State licensure as a Nurse Practitioner or Physician Assistant.
• Graduate of an accredited NP or PA program.
• DEA registration (or ability to obtain).
• Strong clinical assessment and communication skills.
• Ability to work independently while collaborating closely with physicians.

Responsibilities

• Evaluate, diagnose, and manage patients with arterial and venous disease in the outpatient setting.
• Perform comprehensive vascular assessments and develop treatment plans in collaboration with vascular surgeons.
• Provide pre-operative and post-operative evaluations and follow-up care.
• Review vascular ultrasound and diagnostic testing results.
• Coordinate care with referring primary care physicians and specialists.
• Educate patients regarding vascular disease prevention, treatment options, and lifestyle modifications.
