Job summary

Community Care Physicians is seeking an experienced Nurse Practitioner or Physician Assistant to join Vascular Health Partners, a premier vascular practice in the Capital Region of New York. You will work with four board-certified vascular surgeons and a team of APPs in an outpatient setting focused on complex arterial and venous disease.

The role features a four 10-hour day schedule (4-10), with 32 patient-care hours and 8 administrative hours weekly, no overnight hospital rounds, and

Qualifications

• Current New York licensure as NP or PA required.
• Graduate of an accredited NP or PA program.
• DEA registration (or ability to obtain) required.
• Strong clinical assessment and communication skills.

Responsibilities

• Evaluate, diagnose, and manage arterial and venous disease in outpatient settings.
• Perform vascular assessments and develop treatment plans with surgeons.
• Manage PAD, carotid disease, aneurysms, DVT, and venous disease.
• Provide pre/post-operative evaluations and follow-up care.
• Review imaging and diagnostic results and coordinate care with PCPs.
