Job summary

The role emphasizes independent clinical judgment, coordination of care, and collaboration with physicians and the care team; travel to outreach clinics may be required. Eligible candidates must hold PA or CRNP licensure and CPR certification, with transplant nephrology experience preferred.

Qualifications

• Active PA license (NCCPA) or CRNP certification required.
• Graduates of accredited PA program or CRNP program, with transplant nephrology exposure preferred.
• Current CPR certification and PA/CRNP licensure in Pennsylvania.

Responsibilities

• Provide patient-centered outpatient care in Transplant Nephrology.
• Coordinate transitions of care and referrals to specialists.
• Triaging calls and supporting outreach clinics.
• Establish collaborative relationships with physicians and teams.
