Job summary

Brentwood Hospital in Shreveport, LA seeks a Counselor/Social Worker to join a multidisciplinary team providing psychosocial assessments, therapy, and discharge planning for adults with psychiatric and related conditions. This role requires LPC or LCSW licensure and supports families and caregivers throughout treatment.

Qualifications

• Master's Degree in social work, counseling or related field.
• LPC or LCSW licensure required.
• Experience in inpatient or outpatient mental health settings preferred.

Responsibilities

• Conduct psychosocial assessments to document history and identify treatment focus.
• Provide group and family process-oriented therapies and educational groups.
• Daily progress notes documenting progress toward treatment goals and family/agency contacts.
• Develop and coordinate an individualized discharge plan with the treatment team and community resources.
