Job summary

Clarvida is seeking an Outpatient Therapist – Bachelor’s/Master’s Level (Mental Health Worker) to provide individual and family therapy within outpatient/school-based settings in Pennsylvania. You will handle intake assessments, treatment planning, and timely documentation in the electronic health record while coordinating with the clinical team.

Qualifications

• Proficient in conducting psychosocial assessments at intake and follow-ups.
• Ability to develop individualized treatment plans in line with regulations.
• Provide outpatient therapy in the recommended settings as per treatment plans.
• Proficient in completing clinical documentation in the EHR system.

Responsibilities

• Perform psychosocial assessments at intake and during ongoing intervals.
• Develop and implement individualized treatment plans.
• Provide individual and family therapy per consumer needs.
• Complete progress notes, discharge summaries and essential documentation in the EHR.
• Collaborate with team members to support the whole-person approach.
