Summary

This position is responsible for the provision of individual and family therapy sessions for consumers assigned to each caseload. This position also ensures completion of required clinical documentation related to individual caseloads in a timely manner.

Essential Duties and Responsibilities

• Demonstrates proficiency with assessment skills by completing psychosocial assessments at intake and ongoing intervals as outlined by program requirements, completing standardized assessments and/or outcome based measures.
• Develop individualized treatment plans by adhering to applicable county, state, and federal regulations.
• Provides therapy to consumers in the setting recommended by their treatment plans.
• Treats the whole person by collaborating with all team members involved with the consumer.
• Evaluates and reviews individualized treatment plans and assesses the efficacy of treatment through ongoing evaluation.
• Devises transition and discharge planning by evaluating recovery and progress through treatment goals and symptomology.
• Completes all required clinical documentation, including progress notes, discharge summaries, incident reports, and treatment reviews via the electronic health record system.
• Responds to emergency/crisis situations with team members and consumers by ensuring safety and employing de‑escalation skills when needed.
• Maintains an assigned schedule per program guidelines and works to provide regular services.
• Coordinates care through routine conversations and referrals as needed (e.g., natural supports, outside referrals, PCPs, school districts).
• Supports ongoing and future business needs by completing assigned duties, including participating in supervision, training, and meetings as outlined by policies and procedures.
• Adheres to professional and ethical interaction policies, maintaining appropriate boundaries with consumers, families, and coworkers.
• Completes additional duties as assigned to support business priorities.

Required Education and/or Experience

• Master’s degree from an accredited institution in Psychology, Social Work, Counseling, or a similar clinical discipline.
• Verified clinical practicum proper.
• Or licensed (LPC, LCSW, LMFT) in the state of Pennsylvania.

Preferred Education and Experience

• At least 1 year of experience with a CASSP service system or employment by a licensed mental health services agency, or experience subcontracted to a mental health agency, or other clinicians trained in behavioral modification techniques.

• Language Skills: Ability to read, analyze, and interpret complex documents; respond effectively to sensitive inquiries or complaints.
• Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
• Reasoning Ability: Ability to make sound decisions, support consumers calmly and timely, utilize various therapeutic modalities based on consumer needs, and react quickly to prevent injury.
