Minimum Qualifications :

  • Master's degree in Social Work from an accredited college or university; LMSW from Michigan Department of Consumer and Industry Services or LPC accepted.
  • Valid driver's license and auto insurance.
  • Knowledge of co-occurring disorders (mental health and substance abuse).

Job Responsibilities :

  • Engage individuals with empathy, even during acute phases.
  • Screen for co-occurring mental health and substance use disorders; conduct assessments; evaluate interactions between disorders and life events; demonstrate knowledge of diagnostic criteria.
  • Provide short-term therapeutic interventions, crisis intervention, individual, and group therapy; monitor progress.
  • Perform case management, liaise with community agencies, develop treatment plans, and provide educational and consultative services.
  • Gather information for clinical history, document treatment plans and progress, participate in team meetings.
  • Develop individualized, person-centered treatment plans for SA, MH, and COD with specific goals and interventions.
  • Assist in identifying triggers for relapse and develop relapse prevention plans.
  • Provide education to individuals, families, and support persons about mental health and substance use disorders, recovery phases, and treatment options.
  • Coordinate with other providers to ensure continuity of care, perform discharge planning, and support medication management.
  • Complete all required paperwork per Medicaid and Department of Community Health guidelines.
  • Stay informed about Medicaid regulations, CARF standards, DSM V, and eligibility requirements.
  • Perform crisis interventions as needed, support trauma-informed care, and assist individuals with trauma-related issues.
