Clarvida seeks a licensed mental health professional to provide individual and family therapy. You will manage a caseload, complete timely clinical documentation, and collaborate with the care team across home, school, and clinic settings.

Qualifications

  • Master’s degree in a clinical field from an accredited program.
  • Licensure in Pennsylvania (LPC, LCSW, or LMFT) or eligible.
  • Proficiency with psychosocial assessments and treatment planning.
  • Experience with EHR systems and clinical documentation is preferred.

Responsibilities

  • Conduct psychosocial assessments at intake and follow-ups using standardized measures.
  • Develop individualized treatment plans compliant with regulations.
  • Provide therapy per treatment plans across settings.
  • Collaborate with the care team to support the consumer.
  • Monitor and update treatment plans based on progress.
  • Create discharge plans and document outcomes in the EHR.
  • Respond to crises with safety and de-escalation skills.
  • Maintain regular caseload and coordinate care with partners.

Job description

Summary

This position is responsible for the provision of individual and family therapy sessions for consumers assigned to each caseload. This position also ensures completion of required clinical documentation related to individual caseloads in a timely manner.

Essential Duties And Responsibilities
  • Demonstrates proficiency with assessment skills by completing psychosocial assessments at intake and ongoing intervals, including standardized assessments and outcome‑based measures.
  • Develops individualized treatment plans in accordance with county, state and federal regulations.
  • Provides therapy in settings recommended by individual treatment plans.
  • Delivers treatment of the whole person by collaborating with all team members involved with the consumer.
  • Evaluates and reviews the efficacy of treatment plans on an ongoing basis.
  • Develops transition and discharge plans by assessing recovery and progress toward treatment goals.
  • Completes all required clinical documentation, including progress notes, discharge summaries, incident reports and treatment reviews via the electronic health record system.
  • Responds to emergency/crisis situations with team members and consumers, ensuring safety and applying de‑escalation skills when needed.
  • Maintains an assigned schedule to ensure regular service provision and caseload maintenance as agreed with the program supervisor.
  • Co‑ordinates care through routine conversations and referrals to external supports, PCPs, school districts, and other agencies.
  • Participates in ongoing supervision, training, and department meetings to support professional development.
  • Interacts professionally, ethically, and respectfully with consumers, family members and coworkers, maintaining appropriate professional boundaries.
  • Completes additional duties assigned to support business priorities and processes.

Supervisory Responsibilities
  • This position does not have any supervisory responsibilities.
Physical Demands
  • May be required to remain stationary 50% of the time; may be required to move within homes, schools, communities, or offices to provide services.
  • Must operate a computer and other office machinery.
  • May need to ascend/descend stairs, position self in proximity to consumers, and transport resources and materials.
  • Work may occur in a variety of settings, including home, school, community, clinic, and program office environments.
Work Environment
  • The work environment includes home, school, community, or outpatient clinic settings as outlined in individual consumer treatment plans.
Travel
  • Up to 100% travel between appointment sites or other locations as required by program needs.
Required Education And Experience
  • Master’s degree in Psychology, Social Work, Counseling, or a similar clinical discipline from an accredited institution.
  • Verified clinical practicum.
  • Or licensed (LPC, LCSW, LMFT) in the state of Pennsylvania.
Preferred Education And Experience
  • At least one year of experience with a CASSP service system or employment by a licensed mental health services agency or subcontracted agency.

Qualifications
  • Proficiency with electronic health record systems.
  • Proficiency in completing psychosocial assessments.
  • Strong organizational and attention‑to‑detail skills.
  • Clear written and verbal communication skills.
  • Mathematical ability to add, subtract, multiply, and divide using whole numbers and common fractions and decimals.
  • Reasoning ability to make timely, sound decisions supporting consumers and preventing injury.
