Job summary

Nao Medical is seeking a SUD Clinical Social Worker for our Canarsie Clinic in New York City. You will conduct psychosocial assessments, LOCADTR determinations, and provide counseling to patients with substance use disorders within our outpatient program, supported by AI tools that streamline documentation.

In collaboration with CASAC counselors, peer advocates, and prescribing providers, you will coordinate care, support MAT, and develop recovery-focused plans while adhering to NY state

Qualifications

• Master's Degree in Social Work (MSW) from an accredited institution.
• Active New York State license as a Licensed Clinical Social Worker (LCSW).
• Direct experience treating or supporting patients with substance use disorders is required.

Responsibilities

• Assess psychosocial and substance use needs and conduct LOCADTR determinations.
• Provide individual and group counseling using evidence-based modalities.
• Coordinate care for patients on MAT with prescribing providers.
• Build recovery-focused care plans and connect patients to treatment, housing, and community resources.
• Collaborate with CASAC counselors and peer advocates to keep patients engaged in care.
• Maintain accurate records meeting OASAS and NY State requirements with AI-assisted documentation.
