Job summary

St. Luke’s University Health Network is seeking an Outpatient Care Manager, Social Worker to assess and support patients and families in outpatient settings, occasionally inpatient, addressing bio-psychosocial needs and coordinating care with the care team.

The role emphasizes psychosocial assessment, care planning, community resource linkage, and collaboration with physicians, nurses, and other providers to promote self-management and prevent hospitalizations.

Qualifications

• MSW preferred; bachelor's degree with 5 years medical social work experience may be acceptable.
• State licensure for MSW in PA preferred; State licensure for MSW in NJ required; State certification for BSW in NJ required.
• MSW with minimum two years of medical social work case management experience or related field preferred.

Responsibilities

• Provides assessment, care planning and intervention to patients and caregivers, including psychosocial and resource evaluation and planning.
• Provides counseling to help patients/caregivers cope with illness and its social/emotional impact.
• Collaborates with providers, nurses and the health care team to ensure coordinated care.
• Investigates insurance benefits and community resources to provide referrals based on patient/caregiver needs.
• Organizes patient care meetings with internal/external team members and the patient/caregiver to evaluate progress and resolve problems.
• Provides patient/caregiver education related to government mandates/laws.
• Collaborates with patient/caregiver, care team and community partners to address biosocial needs to prevent unnecessary hospitalizations.
• Maintains clinical documentation and required reporting in the department’s MIS.
• Acts as liaison to community agencies to address systems issues affecting patient outcomes.
• Demonstrates competency in assessment, treatment range, and development appropriate to patient age.
