Job summary

Qualifications

• Graduate of an accredited Registered Nurse program.
• Current Michigan RN license and CPR certification.
• 1–2 years clinical experience in medical/physician office preferred.

Responsibilities

• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance and patient education, and coordinate care.
• Support transitions of care and post-discharge follow-up.
• Collaborate with providers, care managers, pharmacists, and other healthcare partners.
• Manage med refills, test results, referrals, and prior authorizations.
• Identify preventive care needs and assist patients in accessing resources.
• Serve as a clinical resource and leader within the care team.
