Trinity Health IHA Medical Group in Michigan is seeking a compassionate Registered Nurse to join our outpatient care team. You will perform clinical triage by phone and in-office, provide care coordination, patient education, and support that improves outcomes and the patient experience.

You will collaborate with providers, care managers, pharmacists and other health partners; manage medication refills, test results, referrals, and prior authorizations; identify preventive care needs and assist

Qualifications

• Graduate of an accredited RN program.
• Valid RN license in Michigan.
• Current CPR certification.

Responsabilités

• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance, patient education, and care coordination.
• Support patients through transitions of care and post-discharge follow-up.
• Collaborate with providers, care managers, pharmacists, behavioral health specialists, and other healthcare partners.
• Manage medication refills, test results, referrals, and prior authorizations.
• Identify preventive care needs and support chronic disease management.
• Help patients access community resources and navigate barriers to care.
• Serve as a clinical resource and leader within the care team.
