Qualifications

• Graduate of a nursing program approved by the appropriate accrediting body (ACEN/CCNE).
• Current, full, active RN license in a U.S. state or territory.
• Preferred outpatient experience and at least one year as an RN.

Responsibilities

• Provide nursing care in multiple clinics including HBPC, infusion, and primary care in the PACT model.
• Coordinate patient care across the hospital, HBPC, and Community Based Outpatient Clinics (CBOCs).
• Educate patients and families on care plans, medications, and self-management.
• Collaborate with an interdisciplinary team to ensure safe transitions across the health care continuum.
• Assess patient conditions, document outcomes, and adjust care plans accordingly.
• Adhere to VA nursing practice standards and participate in quality improvement activities.
