RENALCARE ASSOCIATES S C seeks an experienced Registered Nurse (RN) to join our Ottawa clinic. This full-time role emphasizes nursing judgment, patient relationships, and chronic care management without night or weekend shifts.

You will coordinate care, educate patients, and serve as a critical link between patients and physicians while maintaining accurate medical records and ensuring follow-up care.

Qualifications

• Current RN license in Illinois.
• Minimum nursing experience required.
• Excellent communication and organizational skills.
• Ability to manage changing priorities in a busy clinical environment.
• Strong nursing judgment and escalation skills.
• Experience in nephrology or related clinical areas is a plus.

Responsibilities

• Triage patient calls, assess symptoms and clinical needs.
• Coordinate physician orders, medications, testing, referrals and follow-up care.
• Review clinical information and communicate concerns to providers.
• Provide patient education regarding medications and treatment plans.
• Communicate with physicians, patients and the clinical team.
• Maintain accurate documentation in the electronic medical record.
• Help patients navigate ongoing care and ensure follow-through.
