Job summary

Urban Indian Center of Salt Lake is seeking a skilled Registered Nurse (RN) to deliver advanced outpatient nursing care, lead care coordination, and advance patient safety, immunization services, and clinical quality across the clinic.

Reporting to the Medical Director, you will mentor staff, support preventive services, track follow‑ups, and help ensure compliant documentation and trauma‑informed, culturally responsive care for American Indian and Alaska Native communities.

Qualifications

• Active Utah RN license and current BLS certification.
• Minimum 5 years of direct patient care in outpatient or primary care.
• Experience in immunizations, wound care, and patient education.
• Strong care coordination and clinical leadership.

Responsibilities

• Provide professional outpatient nursing care within the RN scope of practice, including assessment, immunizations, wound care, and patient education.
• Lead interdisciplinary care coordination before, during, and after patient encounters.
• Ensure closed-loop follow-through for lab, imaging, referrals, and preventive-care gaps.
• Mentor staff, support clinical workflows, and promote evidence-based patient care.
