Job summary

Texas Health Resources Rehab Counselor Social Worker in Fort Worth offers full-time 40-hour weeks in the Outpatient Rehab department with a Monday–Friday schedule and no weekend shifts.

You will provide individual, family, and group psychotherapy, perform psychosocial assessments, coordinate with the healthcare team, and help with discharge planning, using CBT and other brief therapies as appropriate.

Qualifications

• Master's Degree in Counseling or Social Work required.
• 2 years of healthcare-related counseling preferred.
• LPC / LCSW / LMSW upon hire required; BCLS prior to independent patient care.

Responsibilities

• Provides individual, family, and group psychotherapy and counseling.
• Conducts psychosocial assessments to determine patient and family needs.
• Coordinates patient care with physicians and other disciplines.
• Assists with discharge planning and transition planning for patients.
