PURPOSE AND SCOPE:

The professional registered nurse Outpatient RN CAP 1 is an entry level position in the Clinical Advancement Program (CAP). This role involves providing and coordinating clinically competent care, including assessment, planning, intervention, and evaluation for assigned patients. It may include delegating tasks to care staff such as RNs, LVNs/LPNs, and Patient Care Technicians. As part of the End Stage Kidney Disease (ESKD) healthcare team, this position contributes to decision-making, teaching, leadership, and quality improvement activities to enhance patient outcomes and facility operations.

PRINCIPAL DUTIES AND RESPONSIBILITIES:
  • Perform all duties under the supervision of the Supervisor and with guidance from the Clinical Educator, Preceptor, or collaborating RNs.
  • Collect and analyze patient data before, during, and after hemodialysis treatments, document findings, and adjust treatment plans accordingly. Notify relevant team members as needed.
  • Assess and document patient and family learning needs, providing education as needed.
  • Coordinate with care staff to ensure safe and effective delivery of hemodialysis therapy.
  • Administer medications as prescribed, documenting medical justification and effectiveness.
  • Assist with emergency response measures when necessary.
  • Serve as a resource for the healthcare team, participate in staff training, and assist in onboarding new staff.
  • Ensure proper laboratory procedures are followed, reschedule missed collections.
  • Update nursing assessments and care plans collaboratively with the interdisciplinary team.
  • Provide patient education on transplant and treatment options.
  • Complete CAP requirements for advancement.
  • Perform other duties as assigned by the Supervisor.
PHYSICAL DEMANDS AND WORKING CONDITIONS:

This role involves heavy lifting, patient movement, and standing for prolonged periods. Equipment aids and assistance may be provided. The environment is typical of a healthcare facility, with exposure to infectious materials possible. Travel to training or other facilities may be required.

SUPERVISION:

Oversight of care staff as a Team Leader or Nurse in Charge after meeting training and experience requirements (minimum 9 months RN experience, 3 months in dialysis).

EDUCATION and LICENSURE:
  • Graduate of an accredited Nursing program.
  • Current state licensure.
  • CPR/BLS certification.
EXPERIENCE AND SKILLS:
  • Entry-level for RNs with less than 2 years of nephrology nursing experience.
  • Dialysis experience preferred.

ADDITIONAL INFORMATION:
