*Annual Salary: $70k-$100k (varies depending on years of experience)*

We are seeking a compassionate, bilingual Outpatient Registered Nurse (RN) to join our client’s dedicated care team. This RN will support patients with chronic illnesses, diabetes, and complex care needs in an outpatient setting. The ideal candidate is fluent in Spanish or Portuguese, culturally competent, and experienced in med-surg, inpatient, or chronic disease management.

You will play a critical role in improving health outcomes by offering education, triage, and ongoing support to patients navigating complex care plans.

*Key Responsibilities:*
* Deliver nursing care to adult patients in a fast-paced outpatient clinic setting
* Conduct assessments, triage concerns, and collaborate with interdisciplinary teams to support care plans
* Educate patients on managing chronic illnesses such as diabetes, hypertension, and heart disease
* Monitor patient progress, provide medication teaching, and help reduce avoidable hospitalizations
* Assist with wound care, vaccinations, and follow-up care as needed
* Maintain accurate documentation in the electronic health record (EHR)
* Serve as a key point of contact for patients and families — in English and Spanish or Portuguese
* Participate in team huddles, case conferences, and care coordination activities

*Qualifications:*
* Current RN license in NJ required
* Fluency in Spanish or Portuguese is required
* 2+ years of nursing experience, preferably in med-surg, inpatient, or chronic disease management
* Strong communication, organizational, and patient advocacy skills
* EMR proficiency (e.g., Epic, eClinicalWorks, etc.) preferred
