Job summary

Advocate Health is seeking a Physical Therapist to evaluate, plan, direct, and administer rehabilitation programs to restore function and prevent disability. Pelvic Floor PT experience is preferred; NC license is required, with a 40-hour work week in a full-time clinical setting.

You will plan and conduct PT programs to enhance physical and functional abilities, collaborate with a multidisciplinary team, and ensure proper documentation and discharge planning.

Qualifications

• Bachelor’s degree is required and NC PT license is required.
• BLS certification required per policy guidelines.
• Pelvic Floor Physical Therapy experience preferred.

Responsibilities

• Evaluates patients and develops comprehensive care plans with modalities and education.
• Gathers data from medical records, physician reports, and patient/family to inform care.
• Establishes functional goals and provides evidence-based interventions with ongoing assessment.
• Documents evaluations, progress notes, and discharge plans; communicates with physicians and staff.
• Supervises PTA's and Rehab Tech/Aide as required for licensure.
