Job summary

Qualifications

• Licensed Physical Therapist in Michigan with active licensure.
• Current CPR certification required.
• Experience in outpatient PT settings and patient‑centered care preferred.

Responsibilities

• Consult with patients to assess physical condition and develop treatment plans.
• Build rapport to motivate patients through therapy programs.
• Coordinate with physicians and health‑care team for continuity of care.
• Document evaluations, progress notes, and discharge summaries in EMR.
