Job summary

H2Health seeks a licensed Physical Therapist to provide one-on-one outpatient care across diverse patient populations. You will evaluate patients, develop and implement individualized treatment plans, and use therapeutic exercise, manual therapy, and modalities to improve mobility, function, and quality of life.

You will collaborate with an interdisciplinary team, document progress, educate patients and families, and contribute to program development.

Qualifications

• Licensed Physical Therapist with active license in the United States.
• Experience delivering outpatient PT services.
• Ability to design and implement individualized treatment plans.
• Proficiency in documenting patient progress using standard EMR tools.
• Strong collaboration within interdisciplinary teams and with families.
