Job summary

Brooks Rehabilitation in Palm Coast, FL is seeking a skilled Physical Therapist to join our Outpatient team on a part-time basis. You will evaluate patients, design individualized treatment plans, and deliver high-quality care across multiple rehab diagnoses in a collaborative, evidence-based environment.

The role offers mentorship, ongoing education, and opportunities for growth within a respected rehabilitation network. Florida licensure and CPR/BLS are required to practice in this setting.

Qualifications

• Current PT license in the state of Florida.
• Current hands-on CPR/BLS Certification.
• Knowledge of federal and state professional requirements.

Responsibilities

• Complete the new patient evaluation, establish customized treatment plans and patient goals, and participate in the implementation of patient care programs.
• Deliver high quality patient care for multiple rehabilitation diagnoses.
• Provide a positive patient experience through patient engagement and progressive treatment.
• Effectively communicate both verbally and written.
• Promote evidence-based patient care.
