Job summary

Thomas Chittenden Health Center in Vermont's Williston area is seeking a dedicated Primary Care Provider (MD, DO, NP, or PA) to join our private practice. This role offers either full-time or part-time hours in an outpatient setting, focusing on comprehensive care and ongoing patient relationships.

You'll collaborate with a multidisciplinary team, maintain EMR documentation, and contribute to quality improvement while supporting a patient-centered, community-focused approach that values autonomy

Qualifications

• Licensed or eligible for Vermont licensure.
• Experience in outpatient primary care preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Provide preventive, acute, and chronic care for patients of all ages.
• Collaborate with a multidisciplinary team to ensure continuity of care.
• Maintain accurate and timely documentation in our EMR system.
• Participate in quality improvement initiatives and practice development; foster a welcoming environment for patients and staff.
