Job summary

Northwell Health Physician Partners in Ridgefield, CT is seeking a BC/BE Internal Medicine/ Pediatrics or Family Practice physician for an outpatient primary care role in a modern 12,000 sq ft facility. The team includes IM/Peds and FP physicians with embedded behavioral health support and care managers to coordinate care.

Qualifications

  • MD or DO with CT State License, Federal DEA and State CS Registration.
  • Completed Internal Medicine/Pediatrics or Family Practice Residency.

Responsibilities

  • Provide comprehensive outpatient primary care for adult and pediatric patients.
  • Collaborate with multidisciplinary team including behavioral health clinicians and care managers.
  • Manage acute and chronic conditions with emphasis on preventive care and wellness.
