Job summary

BAS HealthCare in Plainville, CT is seeking Advanced Practitioners for a 100% outpatient primary care practice focused on adults (18+). Open to NP and PA candidates, you will carry your own patient panel and deliver patient-centered care in a collaborative team.

The role requires an unrestricted CT medical license (or eligibility) with 2+ years of outpatient experience preferred; new graduates are welcome. Bilingual Spanish skills are a plus; we offer market-leading compensation, CME time off,

Qualifications

• Unrestricted CT medical license or eligible.
• APRN/PA training in family, adult or primary care.
• 2+ years outpatient primary care experience preferred.
• New graduates welcome; Spanish-speaking skills a plus.

Responsibilities

• Provide 100% outpatient primary care for adults 18+.
• Carry your own patient panel and develop care plans.
• Perform routine exams and coordinate referrals as needed.
• Collaborate with a supportive, multidisciplinary team.
