Job summary

Circle the City’s Downtown Family Health Center in Phoenix, AZ is seeking an Outpatient Physician to join a dedicated team serving patients experiencing homelessness. The physician will deliver compassionate, patient-centered outpatient care, perform examinations, and develop treatment plans within an integrated care model.

The role requires an unrestricted Arizona medical license, Board-Certified in Family Medicine or Internal Medicine (or BE within 2 years post-residency), and proficiency with

Qualifications

• Must have valid unrestricted Arizona Medical License.
• Board-Certified in Family Medicine or Internal Medicine OR BE within 2 years post residency.
• Certified in Basic Life Support (BLS).
• Experience with homeless or underserved populations preferred.
• Proficient in electronic health records (EHR).

Responsibilities

• Develops care plans and coordinates with the care team.
• Interviews patients, performs exams, and updates medical histories.
• Generates differential diagnoses and management plans for illnesses.
• Provides outpatient care for patients experiencing homelessness.
• Maintains HIPAA-compliant documentation and uses the EMR effectively.
