Outpatient Physical Therapist - Roseville, CA

Join our dedicated team in Roseville, CA as a full-time Outpatient Physical Therapist, where you will have the opportunity to provide transformative care and support to the elderly population. This role focuses on delivering restorative and rehabilitative services both in our facility and in home settings, working closely with a multidisciplinary team to promote maximum independence and functionality in our patients. Salary: $95,000.00 - $115,000.00 per year

Essential Duties and Responsibilities:

• Conduct thorough Physical Therapy assessments at the facility and in patient's homes to determine care needs and set goals.
• Participate actively in interdisciplinary team (IDT) meetings to develop comprehensive care plans for participants.
• Implement Physical Therapy interventions as per physician orders and agreed upon in care plans.
• Perform various treatments including active and passive exercise, muscle re-education, and functional ADL training, utilizing modalities like massage, heat, and electricity.
• Supervise and train Therapy Assistants and staff in the implementation of Functional Maintenance Programs.
• Provide education and training to staff, participants, and caregivers on the use and care of therapeutic modalities and equipment.

Minimum Position Qualifications:

• Bachelor’s Degree in Physical Therapy with a validity of minimum five (5) years of relevant experience.
• Must hold a valid California Physical Therapy License or have proof of License eligibility.
• Experience working with frail or elderly populations in a clinical setting is highly preferred.
• Physical Therapy degree obtained outside of the US should meet US equivalency standards.
• Current CPR certification or ability to obtain prior to employment commencement.

Preferred Qualifications:

• Master’s Degree or Doctorate in Physical Therapy.
• More than two years of clinical experience specifically with an elderly patient demographic.

• Expertise in designing and implementing comprehensive treatment plans.
• Strong interpersonal and communication skills to effectively collaborate with a multidisciplinary team.
