BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a full-time outpatient home health position. The role involves providing home visits for senior living clients in the Harrisburg, PA area, servicing adult and geriatric populations across Lancaster County.

Applicants must have at least one year of recent clinical experience as a licensed Physical Therapist. In this role, you will be a key part of a multidisciplinary team delivering rehabilitative care and skilled nursing, enabling clients to remain at home with appropriate medical support.

Responsibilities:

1. Conduct outpatient visits within designated territories.
2. Perform diagnostic tests such as mobility assessments, strength testing, balance evaluations, and safety assessments.
3. Develop and update individualized care plans, including manual therapy, gait training, and balance interventions.
4. Collaborate with nursing staff to revise care plans as needed.
5. Educate clients and their families on rehabilitative activities to promote health and independence.
6. Document all client interactions and interventions accurately using electronic medical records.

Qualifications:

• Current PA Physical Therapist license.
• Minimum one year of recent PT experience.
• Graduation from an accredited Physical Therapy program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Proficiency with computers; EMR experience preferred.
