$3,000 Sign-On Bonus for Full-Time

BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for a full-time opportunity performing outpatient home health visits for our senior living office located in Wilmington, DE. This office services adult and geriatric clients on a per-visit basis in Independent and Assisted Living Communities throughout the Hockessin area.

One year of prior clinical experience as a licensed Physical Therapist is required. As a home care Physical Therapist, you will be a key member of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing to help clients remain at home.

Responsibilities include:

1. Make outpatient visits to clients in designated geographic areas.
2. Perform diagnostic tests and measurements, including mobility, joint range of motion, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
3. Develop and implement individualized care plans with manual therapeutic exercises, gait training, balance interventions, etc.
4. Assess and revise the care plan regularly, collaborating with nursing as needed.
5. Educate clients and their families on rehabilitative activities to promote health, safety, and independence.
6. Document observations, interventions, and evaluations accurately using a state-of-the-art tablet.

Qualifications:

• Current DE Physical Therapist (PT) license.
• At least one year of recent experience as a PT.
• Graduation from an accredited program approved by the American Physical Therapy Association or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; EMR experience preferred.
