We are seeking an experienced Physical Therapist (PT) for a full-time outpatient home health position, providing visits to senior living communities in the Middletown area. This role focuses on adult and geriatric clients in independent and assisted living settings.

Requirements include at least one year of clinical experience as a licensed Physical Therapist. You will be a vital part of a multidisciplinary team delivering rehabilitative care and skilled nursing, helping clients remain at home with appropriate medical support.

Responsibilities:

1. Conduct outpatient visits within designated territories.
2. Perform diagnostic assessments including mobility, strength, balance, and safety evaluations.
3. Develop and update individualized care plans with interventions like manual exercises, gait training, and balance activities.
4. Collaborate with nursing staff on multidisciplinary care plans.
5. Educate clients and families on rehabilitative activities to promote health and independence.
6. Document client observations and interventions accurately using a touch pad tablet.

Qualifications:

1. Current DE Physical Therapist (PT) license.
2. At least one year of recent clinical experience as a PT.
3. Graduation from an accredited program by the American Physical Therapy Association or equivalent.
4. Ability to work independently and manage time effectively.
5. Strong interpersonal and organizational skills.
6. Proficiency in computer skills; EMR experience preferred.
