Job description

BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for a part-time opportunity performing outpatient home health visits for our senior living office. This office services adult and geriatric clients on a per-visit basis in Independent and Assisted Living Communities throughout the Uniontown area.

As a home care Physical Therapist, you will be an integral member of a multidisciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, enabling them to remain at home with the necessary medical support.

Responsibilities include:

1. Making outpatient visits to clients in designated geographic areas.
2. Performing diagnostic tests and measurements such as mobility, joint range of motion, transfer status, stability, ambulation patterns, muscle strength and endurance, balance testing, and safety assessments.
3. Developing and implementing individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
4. Continuously assessing and revising the care plan, collaborating with nursing staff as needed.
5. Educating clients, family members, and caregivers on rehabilitative care and activities to promote health, safety, and independence.
6. Documenting observations, interventions, and evaluations accurately using a touchpad tablet.

Qualifications:

• Current OH Physical Therapist (PT) license.
• At least one year of recent experience as a Physical Therapist.
• Graduation from an American Physical Therapy Association-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior EMR experience preferred.
